The cardiac rehab program is a supervised and customized outpatient program of exercise and education. We provide a state of the art cardiac program for patients transitioning from acute and subacute inpatient care.
Our team of dedicated professionals oversees the program includes a medical doctor, nurse practitioner and both physical/occupational therapists trained in the specific needs of patients.

The Outpatient Phase II cardiac rehabilitation begins after Phase I (inpatient cardiac rehabilitation). A doctor’s referral is required. Phase II cardiac rehab is a monitored session of individualized, ECG telemetry monitored exercises two to three hours sessions per week for two weeks.. Phase II also includes group and individual education about the cardiovascular risk factors and nutritional guidance in adopting a heart healthy diet.
Phase III – Outpatient Maintenance
Our Phase III Maintenance program is offered through our Fitness & Wellness Center . It includes an exercise regimen supervised by our experienced physical therapist. During Phase III patients will continue to receive exercise supervision but will no longer receive telemetry monitoring.
